The “Terms and Conditions (SMS Terms of Service)” applies to Ali O.D. and Patel O.D., Professional Optometric Corporation” (“the practice”), a California professional optometric corporation. The name may appear with variations, with or without punctuation, and may include the abbreviation “POC” (for Professional Optometric Corporation), with or without punctuation. All such names and variations refer to the same legal entity, Ali O.D. and Patel O.D., Professional Optometric Corporation, which is the covered entity as it pertains to the text message policy. Throughout these Terms and Conditions, the terms “we,” “us,” “practice,” “the practice” and “our practice” refer to this entity.

MESSAGE FREQUENCY

You may receive up to 5 messages per month per person per appointment. For recall or other notices you may receive up to 4 messages per month.

Message and data rates may apply based on your mobile carrier’s terms.

PRIVACY POLICY

Your information will be handled in accordance with our SMS OR Text Message Privacy Policy and our Notice of Privacy Practices.

OPT-OUT INSTRUCTIONS
